How about an Akimbo pickup item?

There was once a doual wileding system in works, I don't remember what's it's status now.

One problem is it needs new player model animations to look right. This is much more possible to do in Xonotic tough (as compared to nexuiz), as we do have the source for our player models in a open format (.blend). It does, however, need someone with blender animation skills to make it happen.

For the code part - yes there was some sorta mutator for this a while back. it would not be to difficult to code a new one either i think. The larger question is how should it work (only 2x same wep, or any combination? Does it affect spread? etc) and if/how it fits in with the game balance.

Not neccessarily, in the beginning it would be enough to make the player model glow in a "quad damage" fashion (e.g. dim orange).

Only two identical weapons, same spread, twice the fire power and twice the need for ammo (balance), secondary fire is disabled while akimbo'ing (balance). Instead, you quickly drop the second weapon when secondary fire is pressed (just in case people don't like it, or in e.g. CTF or TDM).

I remember the Akimbo Arena mutator for UT - you could have 2x the same weapon, but it was handled as if you really had two of the same weapon, each shot from each one was counted and the damage was the same, only it was firing most of the weapons the second one with a time offset of half the fire time of the first one, so in effect you had 2x the fire rate (was fun for slow weapons like shock rifle and sniper). Flak cannon was ultra deadly then ;-).
