Particles will drop your frame rate like a bad habit as will reflections and HDR. Geometry detail and anti-aliasing should be set pretty low in most cases I've seen because I never notice the extra detail the give when playing. Decals can slow things down if they don't fade or disappear quickly enough as well and it's also a good idea to reduce the gib time Cvar (forgot what is was called) that's in the advanced options menu. There's also an LOD Cvar somewhere (forgot where at though) in the advanced options you may want to tweak which will reduce model quality at a distance. One other thing I do is cap the frame rate to either the lowest I think it's going to drop or close to it. Doing that keeps the game running consistently.
